Case Manager, Acute Care

About This Role

This role involves facilitating, coordinating, and managing an assigned caseload to achieve evidence-based clinical outcomes. It requires utilization management, assessment for discharge planning, and ensuring compliance with regulatory and accreditation standards.

Responsibilities

• Facilitate, coordinate, and manage an assigned caseload to promote evidence-based clinical focused outcomes
• Manage utilization management supporting medical necessity, determine appropriate level of care, and resource utilization
• Evaluate the need for services for continuity of care and denial prevention
• Assess, evaluate, and coordinate for safe and appropriate discharge planning needs
• Ensure compliance with state and federal regulatory requirements, TJC accreditation standards, and policies

Requirements

• Associate Degree in Nursing
• Registered Nurse license
• Basic Life Support certification
• 2 years of acute care RN experience

Qualifications

• Associate degree in a Nursing Program from an Accredited School of Nursing
• 2+ years of acute care RN experience
