About This Role

This role involves providing direct support to individuals with intellectual and developmental disabilities in residential or periodic programs, focusing on personal care, skill development, and community integration to help them live well and independently.

Responsibilities

  • Provide direct support or assistance in accordance with individual service or program plans, including socialization and behavioral development
  • Assist with personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, and other daily living activities
  • Accurately document progress and activity, review records, and maintain confidentiality
  • Make cost-effective and appropriate purchases within the budget of the individual served
  • Maintain healthy and professional relationships with individuals, friends, families, guardians, and case managers
  • Report any instance of alleged abuse or neglect, and medical, behavioral, or other incidents according to policy
  • Accompany individuals to medical appointments and relay orders/information to and from medical providers
  • Administer or support self-administration of medication, document delivery, and report errors
  • Monitor individual's health, document concerns, and communicate with nurses or supervisors as appropriate
  • Prepare or support individuals to prepare food in accordance with planned menus and special diets
  • Attend orientation and on-going training, and participate in monthly staff meetings
  • Work scheduled hours, arrive on time, and stay on shift until replacement arrives
  • Transport individuals safely into the community, ensuring proper use of safety equipment
  • Perform daily and seasonal housekeeping, and maintain a neat, clean, and safe environment
  • Monitor environmental safety and perform minor repairs, reporting equipment or facilities requiring repairs

Requirements

  • Must be 18 years of age
  • Working knowledge of computers
  • Valid driver’s license in good standing and meet all insurance requirements
  • Complete all state-required trainings and certifications
  • Submit to drug testing prior to employment and randomly/for cause
  • Submit to criminal background checks and be cleared by Department of Human Services
  • Agree to have name checked against Central Registry of Offenders Against Individuals with Developmental Disabilities and Child Abuse Registry Information (CARI)

Qualifications

  • High school diploma or equivalent preferred
  • Six months of experience in human services preferred.
