This role is responsible for providing comprehensive addiction counseling services to a caseload of clients, supporting their long-term recovery, and ensuring compliance with program and state regulations.
Responsibilities
- Manage a caseload of clients according to program and state requirements
- Complete evaluations and assessments including screening, intake, clinical assessments, ASAM, and biopsychosocial histories
- Develop individual treatment plans upon client admission
- Document all clinical services and interventions in the client's electronic health record within 24-72 hours
- Conduct individual and group sessions based on client treatment plan goals and level of care
- Participate in funding source reviews and provide recommendations for treatment and length of stay
- Monitor client progress and adjust treatment plans as necessary
- Coordinate with agency and community resources to facilitate a comprehensive treatment plan
- Assist clients in developing a discharge/continuing care plan and connecting with resources
- Follow up with discharged clients based on regulatory guidelines until established with other services
Requirements
- DC Health Professional Licensing Administration (HPLA) licensure as CAC, LPC, LGSW, or LICSW
Qualifications
- Bachelor's Degree or Master's Degree in Social Work, Counseling, or related fields
