Provide specialized wound management services to patients in skilled nursing and long-term care facilities, focusing on geriatric care. This role involves evaluating, diagnosing, and treating acute and chronic wounds, developing treatment plans, and collaborating with interdisciplinary teams to improve patient outcomes.
Responsibilities
- Evaluate, diagnose, and treat acute and chronic wounds, including pressure injuries, diabetic ulcers, venous stasis ulcers, and surgical wounds
- Develop and oversee individualized wound care treatment plans
- Perform wound debridement and other minor procedures as clinically indicated
- Monitor wound healing progress and adjust treatment plans accordingly
- Collaborate with nursing staff, advanced practice providers, and facility leadership to ensure optimal patient care
- Provide education and guidance to clinical staff on wound prevention and best practices
- Maintain accurate and timely medical documentation
- Deliver compassionate, patient-centered care with a focus on the geriatric population
Requirements
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- State-specific license in good standing in Vermont
- DEA license
- Experience in wound care
- Strong clinical judgment and excellent communication skills
- Comfortable working with the geriatric population
Qualifications
- Experience in wound care (long-term care or skilled nursing experience a plus)
