Join a dedicated healthcare team as a Neurology Nurse Practitioner/Physician Assistant, delivering high-quality, personalized care directly to residents in skilled nursing and long-term care facilities. This role involves managing neurological conditions like epilepsy, multiple sclerosis, and Parkinson's disease, significantly impacting patients' lives.
Responsibilities
- Develop and implement individualized treatment plans for patients, including medication management and non-pharmacologic interventions
- Conduct regular follow-up visits to monitor patient progress, adjust treatment plans as necessary, and provide education and support
- Collaborate with physicians and other healthcare professionals to coordinate care and ensure a multidisciplinary approach
- Provide patient and family education on disease management, medications, and lifestyle modifications
- Maintain accurate and timely documentation of patient encounters and treatment plans
- Participate in quality improvement initiatives to enhance patient outcomes and optimize patient care delivery
- Perform comprehensive assessments to accurately diagnose and treat patients with neurological conditions
Requirements
- Minimum 6 months of experience in long-term care or a skilled nursing facility
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- DEA License
- CPR BCLS/BLS certified
- Master's Degree in Nursing
- Completed a degree from an accredited Nurse Practitioner program
- A year of experience in Neurology
- Great organizational and time management skills
- Able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
Qualifications
- Master's Degree in Nursing and a degree from an accredited Nurse Practitioner program
- Minimum 6 months of experience in long-term care or a skilled nursing facility, and 1 year of experience in Neurology
Nice to Have
- Experience in Documenting EMR (Sigma, Visual, PCC, etc.)
