Triage Licensed Practical Nurse, LPN PRN

About This Role

This role involves answering, monitoring, and returning calls and communications from Caris Healthcare patients, working under the supervision of an RN or Physician. The Triage LPN will identify patients with at-risk conditions and support triage processes, recognizing physical, psychosocial, and spiritual aspects of care.

Responsibilities

• Answer, monitor, and return calls/communications related to Caris Healthcare patients under RN supervision or Physician direction
• Recognize physical, psychosocial, and spiritual aspects of care
• Accurately identify patients with at-risk conditions while supporting triage processes
• Work within LPN scope of practice as defined by the Board of Nursing and Caris Healthcare policies
• Address patient calls concerning symptom/side effect management and other patient care after hours in a timely manner
• Ask appropriate questions to assist in collecting/identifying patient's symptom and side effect management inquiries
• Recognize changes in the patient's status and need for care
• Identify urgency of patient symptoms/side effects and address them appropriately to prevent unnecessary emergency department and hospital use
• Respond to emergency situations/calls and coordinate care/EMS response as necessary
• Collaborate with Physicians, Nurse Practitioners, and Registered Nurses to discuss patient care needs
• Complete on-call log and distribute to appropriate Caris branch locations by end of shift
• Provide clear education to patients and caregivers, verifying understanding of plan of care within LPN scope of practice
• Counsel patients, family, and/or caregiver about side effects of medication (within LPN scope), applicable referrals, and community resources
• Assist with obtaining prescription refills if needed
• Perform charting and updating of records in the EMR
• Act as a resource for healthcare team, patients, caregivers, and external agencies/organizations
• Participate in and initiate performance improvement activities as requested

Requirements

• Current Compact state LPN licensure or licensure for TN, SC, MO, VA, GA
• Home Health and or Hospice Experience
• Graduate of an accredited school of Licensed Practical/Vocational Nursing
• 2-years of relevant experience in nursing and/or telephone triage

• 2+ years of relevant experience in nursing and/or telephone triage
