Provide direct physical therapy care to patients, assessing their medical condition and functional capabilities. Develop and administer individualized treatment programs, working collaboratively with a healthcare team to achieve patient goals.
Responsibilities
- Assess referred patients requiring rehabilitation services
- Provide direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation
- Establish and administer a treatment program with specific goals determined by patient's capacity and tolerance under physician direction
- Set realistic and achievable goals for patients
- Document and record the patient's condition
- Educate patients and families in appropriate physical therapy methods
- Develop a plan of care for each physical therapy patient
- Provide skilled physical therapy services/interventions in accordance with physician orders
- Assess patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions
- Collaborate with all disciplines to plan and evaluate team goals for each patient
Requirements
- Must hold a Doctorate degree in Physical Therapy OR a Bachelor's degree in Physical Therapy and have passed the NPTE
- Must have graduated from an accredited school
- Minimum one year's experience
- Current state license as a Registered Physical Therapist / PT or proactively in the process of application for current state licensure
- Current CPR certification
Qualifications
- Master's or Doctorate degree in Physical Therapy OR a Bachelor's degree in Physical Therapy with a passed NPTE
- Minimum one year's experience
