About This Role

Provide physical therapy services and case management across multiple locations in the greater Chicago area for patients requiring rehabilitation. This is a 12-week travel assignment working 40 hours per week.

Responsibilities

Work as PT Case Manager
Provide physical therapy services across multiple coverage areas
Travel required between various locations within coverage area
Case management responsibilities included in position
