Responsibilities

• Return patients to optimal physical function in a skilled nursing facility (SNF) setting
• Facilitate achievement of patient, physician, and therapy goals
• Instruct patients in maintaining achieved level of function after discharge through education, independent exercise programs, and appropriate activity modification

Requirements

• Master’s Degree in Speech-Language Pathology from an accredited college or university
• Active, unencumbered license from the state of practice
• Basic computer skills and ability to navigate various software programs, including service delivery software and electronic health record software
