Social Work Care Coordinator

About This Role

This position provides psychosocial services to patients and families, advocating for their needs and connecting them to necessary resources. It involves clinical and complex care planning for patients receiving care in an outpatient setting.

Responsibilities

• Provide patients and families with emotional support, advocacy, financial information, and social work services
• Serve as a liaison between medical staff and patients/families to minimize stress and maximize coping skills
• Provide psychosocial services to patients, families, and consultation to staff
• Offer condition-specific educational information, resources, and support to patients, families, and caregivers
• Conduct clinical and complex care planning for outpatient patients
• Perform psychosocial assessment, care planning, implementation of appropriate interventions, and evaluation
• Prioritize and assess critical situations while maintaining quality services
• Perform initial assessments on referred patients within departmental timeframes

Requirements

• Working-level knowledge of English
• Ability to obtain and maintain State of Nevada Social Work licensure
• One year experience in social service field or relevant internship
• Proficiency with Microsoft Office Suite (Outlook, PowerPoint, Excel, Word)
• Ability to pass Nevada Division of Public and Behavioral Health background checks

Qualifications

• Appropriate education to obtain and maintain Social Work licensure in the State of Nevada
• 1 year experience in social service field, or completion of an internship program in a healthcare setting or department-specific focus. Healthcare experience preferred.
