About This Role

The Service Coordinator helps individuals with developmental disabilities achieve greater independence and inclusion by managing a caseload, identifying needs, and connecting them with appropriate community resources and support systems. This role involves developing action plans, monitoring service effectiveness, and ensuring accurate documentation.

Responsibilities

  • Manage a caseload of varying ages, disabilities, and case complexity to improve the quality of support and services.
  • Establish strong, trusting relationships through regular contact and planning with individuals and their teams to identify needs and create action plans.
  • Develop community relationships and support systems, linking individuals with resources that match their unique support needs.
  • Research existing resources, develop new resources, and make referrals to collaborating agencies.
  • Prepare Utilization Review requests, service authorizations, and coordinate federal, state, county, and community programs.
  • Monitor and assess the quality, timeliness, and effectiveness of services and support received through ongoing review processes.
  • Complete written case note documentation for all activities for each individual within three business days.
  • Maintain a minimum of 120 TCM billable hours per month.
  • Participate in knowledge enrichment, including completing assigned training, continuing education, and attending conferences/seminars.
  • Accept and willingly carry out special assignments or duties to create team unity.

Requirements

  • Reliable transportation and an acceptable driving record
  • Personal vehicle for work-related assignments
  • Valid driver’s license
  • Proof of personal, valid vehicle insurance
  • Access to reliable internet for telecommuting

Qualifications

  • A Bachelor’s degree from an accredited college or university or Registered Nurse license

Nice to Have

  • Experience working with individuals with intellectual and/or developmental disabilities in a community-based setting
  • Experience in social work, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, or a closely related area
  • Experience with case management and documentation applications (e.g., Setworks, Kronos, Therap)
  • Experience as a member of a multi-disciplinary team in assessing, developing, or implementing individual person-centered plans for persons with intellectual/developmental disabilities
  • Experience utilizing the Microsoft Office Suite
  • Class E Missouri Driver’s license if residing in Missouri
