Assess resident health status, develop and revise care plans, and administer nursing interventions to improve lives in a rehabilitation setting. This role involves critical thinking and communication with the interdisciplinary team, residents, and families.
Responsibilities
- Accurately assess residents' health status, educational, and environmental needs on admission and ongoing
- Communicate significant changes in condition to the appropriate interdisciplinary team (IDT) member
- Document all assessments accurately
- Develop dynamic care plans through an interdisciplinary approach that includes physicians, residents, families, and other IDT members
- Review and revise care plans as residents' needs, problems, and goals change
- Communicate care plans to appropriate IDT members, residents, and families
- Document care plans on admission, cognition change, or regulatory requirements
- Identify necessary nursing interventions relevant to the plan of care, including nursing orders, physician orders, and educational/environmental needs
- Administer medications, treatments, and IV fluids safely and timely
- Document all interventions accurately and timely
- Use critical thinking skills when dealing with residents' change in condition
- Notify the physician timely using the SBARR communication tool
- Maintain composure and handle crisis situations calmly and professionally
- Monitor resident care to ensure effective care plan delivery
- Identify problem areas and use the QAPI process for immediate problem resolution
- Assist in answering call lights and with resident ADLs
- Complete a minimum of twelve hours of nursing-related offerings
Requirements
- Education from an accredited school of nursing that results in becoming a registered nurse
- Current RN license in Illinois
- CPR/AED certification or willing to obtain within six months of employment
- Critical thinking skills
- Excellent communication skills
- Excellent time management and organizational skills
- Ability to stand and walk for long periods
- Ability to lift heavy objects using proper techniques
- Ability to transfer and move patients weighing 200+lbs
- Ability to push and pull equipment weighing over 40lbs
- Manual dexterity, coordination, and skillful use of hands
- Visual acuity with ability to distinguish color
- Ability to physically perform necessary documentation electronically and in writing
Qualifications
- Education from an accredited school of nursing that results in becoming a registered nurse
