This Registered Nurse (RN) position assesses and develops care plans for residents, ensuring high-quality care through effective interventions and communication. The role involves managing resident health status, documenting assessments, and collaborating with an interdisciplinary team.
Responsibilities
- Accurately assess residents' health status, educational, and environmental needs on admission and ongoing
- Communicate significant changes in condition to the appropriate interdisciplinary team (IDT) member
- Document all assessments accurately and timely
- Develop dynamic care plans through an interdisciplinary approach, including physicians, residents, families, and IDT members
- Review and revise care plans as resident's needs, problems, and goals change
- Identify necessary nursing interventions, including nursing orders, physician orders, and educational needs
- Administer medications, treatments, and IV fluids in a safe and timely manner
- Use critical thinking skills when dealing with residents' change in condition and notify physicians using SBARR
- Maintain composure and handle crisis situations professionally
- Monitor resident care to ensure effective care plan delivery and address problem areas using the QAPI process
Requirements
- Education from an accredited school of nursing
- Current RN license in the state of working
- CPR/AED certification or willing to obtain within six months
- Critical thinking skills
- Excellent communication skills
- Excellent time management and organizational skills
- Ability to stand and walk for long periods
- Ability to lift heavy objects using proper techniques
- Ability to transfer and move patients weighing 200+lbs
- Ability to push and pull equipment weighing in excess of 40lbs
- Manual dexterity, coordination and skillful use of hands
- Visual acuity with ability to distinguish color
- Ability to perform necessary documentation electronically and in writing
Qualifications
- Education from an accredited school of nursing that results in becoming a registered nurse
