Provide prescribed nursing care services to ill, injured, convalescent, and disabled persons in a home setting. Collaborate and communicate with the interdisciplinary team to meet hospice organization goals and objectives.
Responsibilities
- Provide prescribed nursing care services to ill, injured, convalescent, and disabled persons in a home setting independently without on-site supervision
- Participate actively in meeting the goals and objectives of the hospice organization
- Collaborate and communicate with members of the interdisciplinary team
- Demonstrate ability to transfer scientific knowledge from social, biological, and physical sciences in applying the nursing process
- Perform accurate assessments and develop/implement written Plan of Care with the patient
- Perform and document nursing interventions including patient education
- Document patient status in an accurate, timely, and concise manner
- Maintain a committed and cooperative attitude with staffing, promoting teamwork and harmony
- Make visits as scheduled and participate in on-call rotation as required
Requirements
- Graduate of an accredited school of nursing
- 2 years experience in nursing
- Valid Oklahoma registered nurse license in good standing
- Current CPR
- Valid driver’s license
- Current automobile insurance
- Reliable vehicle
Qualifications
- Graduate of an accredited school of nursing
- 2 years experience in nursing, preferred 1 year in home health &/or hospice
