The Registered Nurse (RN) Home Health provides in-home patient care, coordinating all disciplines involved, and communicates with physicians. Key responsibilities include accurate OASIS collection, maintaining medication profiles, and timely reporting of lab values.
Responsibilities
- Perform visits and complete coordination of client care
- Act as the point of contact for all disciplines involved in patient care
- Oversee the frequency of visits for each episode
- Consult with physicians and the office regarding patient care details
- Perform accurate OASIS collection
- Ensure the medication profile remains current
- Ensure lab values are reported to the physician timely
- Attend weekly case conferences and monthly case manager meetings
Requirements
- Currently licensed as a Registered Nurse (RN) in the state of employment
- Demonstrated knowledge and skill in current nursing practice
- Possess a valid state driver’s license and automobile liability insurance
Qualifications
- A minimum of one year of clinical experience is preferred
