Registered Nurse, Home Health

About This Role

As a Home Health Registered Nurse, you will deliver personalized, one-on-one care to help patients regain independence in their homes. You will work within a dedicated team of physicians and clinicians to develop and manage comprehensive care plans.

Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
• Confer with physicians to develop initial plans of treatment based on orders and patient assessment.
• Coordinate appropriate care, encompassing various healthcare personnel such as Physical Therapists, Occupational Therapists, and Home Health Aides.
• Report patient care/condition/progress to the patient's physician and Clinical Manager on a continuous basis.
• Implement patient care plans in conjunction with the patient and family to assist them in achieving optimal resolution of needs/problems.
• Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation for all patients, including case conferences, patient contacts, and care plan changes.
• Prepare visit/shift reports, summarize patient records, and confer with other health care disciplines to provide optimum patient care.

Requirements

• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance and reliable transportation

Qualifications

• Diploma, Associate or Bachelor Degree in Nursing
• Minimum of one year nursing experience preferred

Nice to Have

• Strong med surg, ICU, ER, acute experience
• Home Health experience a plus
