Registered Nurse, Home Health

About This Role

As a Home Health Registered Nurse at CenterWell Home Health, you will provide personalized, one-on-one care to patients in their homes, helping them regain independence. You will develop and manage care plans in collaboration with physicians and a dedicated clinical team to support patient recovery.

Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home health patients
• Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager
• Confer with physicians to develop initial treatment plans and revise them based on ongoing assessments
• Coordinate appropriate care involving various healthcare personnel and external providers
• Report patient care, condition, and progress to the patient's physician and Clinical Manager on a continuous basis
• Implement patient care plan in conjunction with patient and family to achieve optimal resolution of needs/problems
• Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers
• Discharge patients after consultation with the physician and Clinical Manager, completing needed clinical documentation
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes

Requirements

• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Valid driver’s license
• Auto insurance and reliable transportation
• Strong organizational and communication skills

Qualifications

• Diploma, Associate or Bachelor Degree in Nursing
• Minimum of one year nursing experience preferred

Nice to Have

• Strong med surg, ICU, ER, acute experience
• Home Health experience
