Registered Nurse, Home Health

About This Role

As a Home Health Registered Nurse at CenterWell Home Health, you will provide personalized, one-on-one care to help patients regain independence in their homes. You will develop and manage care plans in close collaboration with physicians and a dedicated team, supporting patient recovery and well-being.

Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer on-going care and case management for each patient, providing necessary follow-up as directed.
• Confer with physicians to develop initial plans of treatment and revise them based on ongoing assessments.
• Coordinate appropriate care with various healthcare personnel and external providers.
• Report patient care, condition, and progress to the patient's physician and Clinical Manager continuously.
• Implement patient care plans with patients and families to achieve optimal resolution of needs/problems.
• Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
• Discharge patients after consultation with the physician and Clinical Manager, completing all necessary clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, and progress updates.

Requirements

• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Valid driver’s license, auto insurance and reliable transportation
• Strong organizational and communication skills

Qualifications

• Diploma, Associate or Bachelor Degree in Nursing
• Minimum of one year nursing experience preferred. Strong med surg, ICU, ER, and acute experience.
