This role coordinates and oversees all direct care patient services provided by clinical personnel, focusing on home health quality assurance and operations initiatives. You will report to the Branch Director in Marion, OH, contributing to the delivery of quality home care services.
Responsibilities
- Develop, plan, implement, analyze, and organize clinical operations for a specific managed location
- Conduct and/or delegate patient assessment and reassessment, care plan updates, and interpretation of patient needs
- Manage the assignment of caregivers to cases
- Oversee the delivery of care to all patients served by the location, including case referrals and admission decisions
- Instruct and guide clinicians to promote performance and delivery of quality home care services
- Monitor cases for compliance with regulatory agencies and third-party payer requirements, ensuring timely final audits/billing
- Coordinate communication between team members, physicians, and caregivers for appropriate care and outcome planning
- Supervise all clinical employees, including interviewing, hiring, training, development, and performance evaluation
- Assess staff education needs and conduct regular education sessions
- Perform QA/PI activities, working with Utilization Review staff for data tracking and performance analysis
Requirements
- Graduate of an accredited School of Nursing
- Current state license as a Registered Nurse
- Proof of current CPR
- Valid driver's license, auto insurance and reliable transportation
- Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment
- Home health experience is required
- OASIS experience
- Homecare Homebase (HCHB) experience
Qualifications
- Graduate of an accredited School of Nursing
- 2 years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment. Home health experience is required.
