The Clinic Rheumatology and INR RN manages the care of patients with chronic rheumatic diseases, autoimmune conditions, and those receiving anti-coagulation services, ensuring appropriate treatments and follow-up care. This role involves skilled clinical care, patient education, and advocacy in an outpatient clinic setting.
Responsibilities
- Follow the RN process for nursing assessment, diagnosis, planning, implementation of care, and evaluation of patient progress to support providers in the clinic, rheumatology, and INR patients.
- Collaborate with cardiologists, physicians/providers, and pharmacy to manage patients on anti-coagulant therapy according to organizational policies and protocols.
- Work collaboratively with clinic and hospital personnel/departments to support smooth patient transitions.
- Act as a resource and team lead to LPN and CMA staff in assigned unit in accordance with the Minnesota Nurse Practice Act (NPA).
- Provide patient education following clinic visits.
- Manage Rheumatology providers' in-baskets, including phone calls, MyChart messages, refills, and relay test results.
- Complete patient chart preparation, including medication, allergy, problem list, and immunization reconciliation.
- Triage patients over the phone and direct them appropriately with clinical judgment.
- Provide evidence-based patient and staff education relating to clinical care.
- Complete Transitional Care Management (TCM) calls for hospital and emergency department discharged patients, when directed.
Requirements
- Registered Nurse with an active RN license in the state of Minnesota
- 1 year experience in chronic disease management in an outpatient setting
- Knowledge of the Minnesota Nurse Practice Act and the Scope of Ambulatory Care Nursing
- Demonstrated clinical competence in providing direct patient care and patient and family education
- Current BLS certification (or willingness to obtain within two months of employment)
Qualifications
- 1 year experience in chronic disease management in an outpatient setting
