This role coordinates patient care activities for individuals requiring enhanced care coordination, ensuring they receive timely, high-quality healthcare and support services. The coordinator utilizes care protocols, care plans, and referrals to appropriate resources, while promoting teamwork within the healthcare team.
Responsibilities
- Coordinate patient care activities for patients in need of enhanced care coordination
- Ensure patients receive timely and high quality healthcare and support services
- Utilize care protocols, care plans, self-management goals, and referrals to appropriate resources
- Promote teamwork with all members of the healthcare team
- Perform all duties in a manner consistent with St. Luke’s mission and values
Requirements
- Associate Degree or Healthcare Credential (LPN, RN, etc.)
- 3 years of Healthcare Experience
