About This Role

This Physical Therapy Assistant position at Astoria Health and Rehab focuses on restoring residents' optimal physical function and helping them achieve their therapy goals within a skilled nursing facility.

Responsibilities

• Return residents to optimal physical function
• Facilitate achievement of patient, physician, and therapy goals in a SNF setting
• Instruct patients in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification

Requirements

• Associate’s Degree in a Physical Therapy Assistant program from an accredited college or university
• Current, unencumbered license to practice as a Physical Therapy Assistant within the state
• Basic computer skills and ability to navigate through various software programs, including service delivery software and electronic health record software
