Deliver individualized physical therapy programs to older adults and children in various settings, including in-home care. Assess needs, develop treatment plans, and educate patients and caregivers to promote independence and well-being.
Responsibilities
- Primarily provide outpatient rehabilitation services in an in-home setting for the older adult population
- Evaluate and treat in the most functional environment which is the patient's home
- Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation
- Identify the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients
- Implement individualized therapy programs designed to restore, reinforce, and enhance performance
- Instruct, educate and train patients and caregivers in those skills and functions essential for promoting independence and productivity
- Direct activities to promote and maintain health
- Supervise Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards
- Adhere to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings
- Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures
Requirements
- Valid Physical Therapy license in the state(s) of practice
- Master's degree in Physical Therapy or related field OR Bachelor's degree in Physical Therapy with 5 years PT experience
- Valid driver's license or reliable, independent transportation
Qualifications
- Master's degree in Physical Therapy OR Master's degree in a Physical Therapy related field OR Bachelor's degree in Physical Therapy with 5 years progressively responsible PT experience
