Physical Therapist (PT)

About This Role

This Physical Therapist (PT) role involves planning and administering physical therapy services in the home and community to pediatric patients with various disabilities and delays. The position focuses on implementing treatment programs and maximizing the independence of each child.

Responsibilities

• Provide high quality care by performing evaluations, interpreting assessment results, and developing goals appropriate to the child.
• Create and implement physical therapy treatment plans in conjunction with the physician.
• Implement skilled interventions to maximize the potential of each individual child, assisting them in developing or regaining physical, neurological, cognitive, and social/emotional functioning.
• Educate, coach, and empower caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine.
• Coordinate with referral partners to provide services for children in accordance with physician orders.
• Assure continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs.
• Document patient care services and care coordination in an intuitive electronic medical record system.
• Maintain patient confidence by keeping information confidential.

Requirements

• State Physical Therapist license
• Current CPR certification

Qualifications

• A minimum of 1 yr. of experience preferred
