About This Role

This role involves implementing physical therapy treatment programs for pediatric patients with various disabilities, focusing on improving their physical, neurological, cognitive, and social/emotional functioning within a home and community setting. The Physical Therapist will also educate and empower caregivers and families.

Responsibilities

  • Provide high quality care and meet patient and family needs by performing evaluations, interpreting assessment results, developing goals, and creating/implementing physical therapy treatment plans in conjunction with the physician
  • Assist pediatric patients to develop or regain physical, neurological, cognitive, and/or social/emotional functioning and improve their level of independence by implementing skilled interventions
  • Participate in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine
  • Coordinate with referral partners to provide services for children in accordance with the physician order
  • Assure continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs
  • Document patient care services and care coordination in an intuitive electronic medical record system
  • Maintain patient confidence by keeping information confidential

Requirements

  • State Physical Therapist license
  • Current CPR certification

Qualifications

  • A minimum of 1 year of experience preferred

Nice to Have

  • 1 year of experience in physical therapy
