About This Role

This Physical Therapist role involves direct participation in physical therapy treatment for adult and geriatric patients, focusing on enhancing their quality of life. The role requires conducting screenings, developing treatment plans, monitoring progress, and maintaining patient records.

Responsibilities

• Provide screenings and evaluations to determine requirements for skilled physical therapy intervention.
• Develop and implement treatment plans focused on functional and measurable outcomes, adhering to professional standards.
• Monitor patients' progress and adjust treatment plans as needed.
• Provide appropriate supervision for physical therapist assistants in compliance with regulations.
• Maintain accurate and up-to-date patient records.
• Collaborate with, coordinate, and participate in training programs for community staff, patients, families, and rehabilitation staff.
• Participate in continuous quality improvement activities.

Requirements

• Current valid Physical Therapist license to practice in appropriate state
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
• Compassionate and patient-centered approach to care

Qualifications

• Successful completion of an accredited Physical Therapy program
