Physical Therapist

About This Role

The Physical Therapist will provide mobile outpatient rehabilitation services to older adults in their homes or independent living facilities, assessing needs, developing, and delivering individualized physical therapy programs.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for the older adult population
• Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation
• Identify the need for and make referral(s) to other disciplines to address the comprehensive needs of patients
• Implement individualized therapy programs designed to restore, reinforce, and enhance performance
• Instruct, educate and train patients and caregivers in skills essential for promoting independence and productivity
• Direct activities to promote and maintain health
• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards
• Adhere to department and patient treatment schedule, timely clinical and billing documentation requirements
• Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures
• Develops appropriate treatment goals and methods in collaboration with the patient and caregivers

Requirements

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
• Master's degree in Physical Therapy OR Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy OR Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
• Valid driver's license or reliable, independent transportation

Qualifications

• Master's degree in Physical Therapy or related field, or Bachelor's degree in Physical Therapy and 5 years progressively responsible PT experience
