Provide mobile outpatient physical therapy services to older adults in their homes or independent living facilities, assessing needs, developing treatment programs, and promoting independence and healthy living.
Responsibilities
- Provide outpatient rehabilitation services in an in-home setting for the older adult population
- Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation
- Identify the need for and make referral(s) to other disciplines for comprehensive patient needs
- Implement individualized therapy programs designed to restore, reinforce, and enhance performance
- Instruct, educate, and train patients and caregivers in skills and functions for independence and productivity
- Direct activities to promote and maintain health
- Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as needed
- Ensure adherence to department and patient treatment schedule, timely clinical and billing documentation
- Attend and contribute to patient care, staffing conferences, and other related meetings
- Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures
- Develop appropriate treatment goals and methods in collaboration with the patient and caregivers
- Perform routine reexamination as needed to modify/progress plan of treatment
- Perform home and community mobility assessments and provide recommendations for safe mobility
Requirements
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy; or Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation
