Physical Therapist

About This Role

Provide mobile outpatient physical therapy services to older adults in their homes or independent living facilities, assessing needs, developing treatment programs, and promoting independence and healthy living.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for the older adult population
• Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation
• Identify the need for and make referral(s) to other disciplines for comprehensive patient needs
• Implement individualized therapy programs designed to restore, reinforce, and enhance performance
• Instruct, educate, and train patients and caregivers in skills and functions for independence and productivity
• Direct activities to promote and maintain health
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as needed
• Ensure adherence to department and patient treatment schedule, timely clinical and billing documentation
• Attend and contribute to patient care, staffing conferences, and other related meetings
• Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures
• Develop appropriate treatment goals and methods in collaboration with the patient and caregivers
• Perform routine reexamination as needed to modify/progress plan of treatment
• Perform home and community mobility assessments and provide recommendations for safe mobility

Requirements

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
• Master's degree in Physical Therapy; or Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
• Valid driver's license or reliable, independent transportation

Qualifications
