Provide mobile outpatient physical therapy services to older adults in their homes or independent living facilities, focusing on rehabilitation and wellness to improve their quality of life. This role involves assessing patient needs, developing individualized treatment plans, and instructing patients and caregivers.
Responsibilities
- Provide outpatient rehabilitation services in an in-home setting for older adults
- Evaluate and treat patients in their most functional environment (patient's home)
- Assess the need for, develop, and deliver physical therapy programs
- Identify the need for and make referrals to other disciplines to address comprehensive patient needs
- Implement individualized therapy programs designed to restore, reinforce, and enhance performance
- Instruct, educate, and train patients and caregivers in skills for promoting independence and productivity
- Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel
- Ensure adherence to department and patient treatment schedules and timely clinical/billing documentation
- Attend and contribute to patient care, staffing conferences, and other related meetings
- Perform routine reexamination to modify/progress plan of treatment
- Incorporate health and wellness programming into treatment plans
- Perform home and community mobility assessments and provide recommendations for safe mobility
Requirements
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy or a Physical Therapy related field plus a Bachelor's degree in Physical Therapy, or a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation
