Physical Therapist - Mobile Outpatient

About This Role

Provide mobile outpatient physical therapy services to older adults in their homes or independent living facilities. Deliver integrated, individualized solutions focused on rehabilitation and wellness to restore their full potential.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for the older adult population
• Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation
• Identify the need for and make referral(s) to other disciplines to address the comprehensive needs of patients
• Implement individualized therapy programs designed to restore, reinforce, and enhance performance
• Instruct, educate and train patients and caregivers in skills and functions essential for promoting independence and productivity
• Direct activities to promote and maintain health
• Screen, examine and evaluate patients, including history, systems review, and application of appropriate tests and measures
• Develop appropriate treatment goals and methods in collaboration with the patient and caregivers
• Design, fabricate, fit and/or apply adaptive devices, and train patients in the use of adaptive devices
• Perform routine reexamination as needed/required to modify/progress plan of treatment

Requirements

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
• Master's degree in Physical Therapy OR Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy OR Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
• Valid driver's license or reliable, independent transportation
