The Physical Therapist will provide in-home outpatient rehabilitation services to older adults, assessing needs, developing treatment plans, and implementing individualized therapy programs to restore and enhance performance.
Responsibilities
- Provide outpatient rehabilitation services in an in-home setting for the older adult population.
- Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation.
- Identify the need for and make referral(s) to other disciplines for comprehensive patient care.
- Implement individualized therapy programs designed to restore, reinforce, and enhance performance.
- Instruct, educate, and train patients and caregivers in skills and functions essential for promoting independence and productivity.
- Direct activities to promote and maintain health.
- Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel.
- Adhere to department and patient treatment schedule, timely clinical and billing documentation requirements.
- Attend and contribute to patient care, staffing conferences, and other related meetings.
- Adapt program and/or plan of care according to the needs of the individual patient.
- Promote a safe environment and enforce the elimination of fire and safety hazards.
- Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
- Develop appropriate treatment goals and methods in collaboration with the patient and caregivers.
- Perform routine reexamination as needed/required to modify/progress plan of treatment.
- Perform home and community mobility assessments and provide recommendations for safe mobility.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy OR Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy OR Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation
