About This Role

Deliver mobile outpatient physical therapy services to older adults in their homes or independent living facilities, focusing on personalized rehabilitation programs to improve their physical well-being.

Responsibilities

  • Provide outpatient rehabilitation services in an in-home setting for the older adult population
  • Evaluate and treat patients in their most functional environment, including private residences or independent living facilities
  • Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation
  • Identify the need for and make referral(s) to other disciplines to address comprehensive patient needs
  • Implement individualized therapy programs designed to restore, reinforce, and enhance performance
  • Instruct, educate, and train patients and caregivers in skills essential for promoting independence and productivity
  • Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements
  • Adhere to department and patient treatment schedules, and timely complete clinical and billing documentation requirements
  • Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures
  • Perform routine reexamination as needed to modify or progress the plan of treatment

Requirements

  • Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
  • Master's degree in Physical Therapy; or Master's degree in a Physical Therapy related field plus a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
  • Valid driver's license or reliable, independent transportation

Qualifications

  • Master's degree in Physical Therapy or equivalent
  • Entry level with mentorship program support
