Physical Therapist - Mobile Outpatient

About This Role

Provide mobile outpatient physical therapy services to older adults in their homes or independent living facilities, working with a team to facilitate rehabilitation and improve patients' quality of life.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for the older adult population
• Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation
• Identify the need for and make referral(s) to other disciplines to address comprehensive needs of patients
• Implement individualized therapy programs designed to restore, reinforce, and enhance performance
• Instruct, educate, and train patients and caregivers in skills and functions essential for promoting independence and productivity
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as necessary
• Ensure adherence to department and patient treatment schedule, timely clinical and billing documentation requirements
• Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures
• Develop appropriate treatment goals and methods in collaboration with the patient and caregivers
• Incorporate health and wellness programming into treatment plans for patients

Requirements

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
• Master's degree in Physical Therapy (or equivalent combination of degrees/experience)
• Valid driver's license or reliable, independent transportation

Qualifications

• Master's degree in Physical Therapy or a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy, or a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
