Assess patients' strengths and weaknesses, devise creative treatment plans, and provide positive encouragement to help them achieve healing, recovery, and functional independence within a high-energy, positive environment.
Responsibilities
- Assess patients’ strengths and weaknesses
- Devise creative and effective treatment plans
- Maintain patient confidentiality
- Utilize training and skills to aid patients in healing, recovering, improving pain level/tolerance, and maximizing functional independence
- Examine patients using fundamental clinical knowledge
- Implement and execute treatment plans and plans of care daily
- Collect and record patient care data following service guidelines, regulatory agencies, and third-party payers
- Prepare for evaluations by obtaining patient history and researching diagnoses
- Select and administer tests and measures, synthesizing results for diagnosis, goals, prognosis, and plan of care
- Collaborate with interdisciplinary teams, patients, and support systems to establish and modify treatment plans
- Identify need for consultations or referrals to other healthcare professionals
- Initiate and complete discharge plans
- Provide education and training to patients and their support systems on ADLs/IADLs, therapeutic activities, rehabilitation equipment, and community reintegration
- Provide safe and effective musculoskeletal interventions including therapeutic exercise, joint mobilization, and modalities
- Provide data and note writing using Prompt EMR for all treatments
- Oversee support staff including PTAs, ATCs, and PT aides
- Attend annual JAG Billing and Coding seminar
- Promote JAG in the community through lectures and presentations
Requirements
- Bachelor’s or Master’s Degree
- Licensed Physical Therapist in the state practicing in (NY)
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Excellent oral and written communication
- Strong clinical decision-making skills
- Excellent work ethic and dedication to patient success
- Basic computer skills
- Ability to work as a team member and individually with minimal supervision
- Maintain a positive, professional demeanor at all times
Qualifications
- Bachelor’s or Master’s Degree required, Doctorate degree preferred
