Physical Therapist

About This Role

Provide physical therapy services on a per diem basis for home health patients, treating impairments and functional limitations according to professional standards. Collaborate with physicians and other practitioners to develop and implement patient care plans.

Responsibilities

• Assist physicians in evaluating patient function and developing plans of care
• Complete initial and ongoing comprehensive assessments
• Provide interventions and therapeutic treatment per physician-certified plan of care
• Assess changes in patient status and modify services as indicated
• Advise and consult with family and caregivers to promote patient progress
• Educate patient, family, and caregivers on goals
• Communicate with interdisciplinary healthcare team to coordinate care
• Prepare timely, legible, and relevant documentation of patient care
• Supervise professional and paraprofessional staff as assigned
• Perform and prioritize multiple functions and tasks effectively

Requirements

• Current licensure as a physical therapist in the state of New Jersey
• Graduate of an accredited physical therapy education program or equivalent
• 2 years of relevant physical therapist experience in the last 5 years
• CPR certification (within 60 days)

Qualifications

• Graduate of an accredited physical therapy education program or equivalent as recognized by the American Physical Therapy Association (APTA)
• 2 years of experience as a physical therapist relevant to the patient population assigned within the last five years
