Physical Therapist, Home Health

About This Role

As a Home Health Physical Therapist at CenterWell Home Health, you will deliver personalized care to patients in their homes, focusing on rehabilitation and functional improvement to help them regain strength, mobility, and independence. You will plan and administer prescribed treatments for patients suffering from various injuries, illnesses, and functional disabilities.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients to attain the highest level of physical function.
• Test/screen patient's physical strengths to assist physicians in evaluating their level of function and develop treatment programs.
• Develop and implement conditioning/rehabilitation programs consistent with physician's Plan of Treatment and patient goals, adjusting as needed for maximum results.
• Confer with physicians and clinical team members to develop, implement, and revise therapy treatment programs and Plans of Treatment.
• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans, supervise and evaluate their performance, and monitor documentation.
• Accurately, promptly, and thoroughly document patient care observations, interventions, and evaluations, ensuring interim orders are documented and implemented.
• Report patient's progress to the patient's physician, Clinical Manager, staff, patient, and family, submitting evaluations, treatment plans, progress reports, and discharge summaries.
• Supervise and evaluate Physical Therapy Assistants and Home Health Aide staff, ensuring appropriate documentation completion.

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Current and unrestricted Physical Therapy license
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance, and reliable transportation

Qualifications

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Minimum of one year physical therapy experience preferred
