Physical Therapist, Home Health

About This Role

As a Home Health Physical Therapist at CenterWell Home Health, you will provide personalized physical therapy treatment to patients in their homes to help them regain strength, mobility, and independence. This role involves planning and administering prescribed treatments, assessing patient progress, and collaborating with physicians and clinical teams to optimize patient care.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients to attain their highest level of physical function.
• Test and screen patients' physical strengths to assist in evaluating their level of function and developing treatment programs.
• Develop and implement conditioning/rehabilitation programs consistent with physician's Plan of Treatment and patient goals.
• Confer with physicians and clinical team members to develop, implement, and revise therapy treatment programs.
• Provide Physical Therapy Assistants and Home Health Aides with written instructions and care plans, and supervise/evaluate their performance.
• Accurately and promptly document patients' care observations, interventions, and evaluations.
• Report patient's progress to the patient's physician, Clinical Manager, staff, patient, and family.

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Current and unrestricted Physical Therapy license
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance, and reliable transportation

Qualifications

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Minimum of one year physical therapy experience preferred
