Physical Therapist, Home Health

About This Role

As a Home Health Physical Therapist, you will provide personalized skilled physical therapy treatments to patients in their homes, helping them regain strength, mobility, and independence. You will plan, administer, and adjust rehabilitation programs, collaborating with physicians and clinical teams.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients
• Test and screen patient's physical strengths to assist in evaluating functional levels and developing treatment programs
• Develop and implement conditioning/rehabilitation programs consistent with physician's Plan of Treatment and patient goals
• Adjust treatment as needed to achieve maximum results
• Confer with physician and clinical team members to obtain patient information and revise therapy treatment plans
• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions and supervise their performance
• Accurately, promptly, and thoroughly document patients' care observations, interventions, and evaluations
• Report patient's progress to the physician, Clinical Manager, staff, patient, and family

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Current and unrestricted Physical Therapy license
• Current CPR certification
• A valid driver’s license, auto insurance, and reliable transportation

Qualifications

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Minimum of one year physical therapy experience preferred

Nice to Have

• Minimum of one year physical therapy experience
