Physical Therapist, Home Health

About This Role

As a Home Health Physical Therapist at CenterWell Home Health, you will deliver personalized care to help patients regain strength, mobility, and independence in their homes, focusing on rehabilitation and functional improvement.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients
• Test/screen patients' physical strengths to assist physicians in evaluating function levels and developing treatment programs
• Develop and implement conditioning/rehabilitation programs consistent with physician's Plan of Treatment and patient goals
• Confer with physicians and clinical team members to develop, implement, and revise therapy treatment programs
• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions, supervise their performance, and monitor documentation completion
• Accurately, promptly, and thoroughly document patients' care observations, interventions, and evaluations
• Report patient's progress to the patient's physician, Clinical Manager, staff, patient, and family

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Current and unrestricted Physical Therapy license
• Current CPR certification
• Valid driver’s license, auto insurance, and reliable transportation

Qualifications

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Minimum of one year physical therapy experience preferred

Nice to Have

• Minimum of one year physical therapy experience
