About This Role

The Home Health Physical Therapist at CenterWell Home Health provides personalized physical therapy treatment to patients recovering from injuries and illnesses in their homes to help them regain strength, mobility, and independence. This role involves planning and administering treatments, evaluating patient progress, and collaborating with the clinical team.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients to attain the highest level of physical function.
• Test and screen patients' physical strengths to assist physicians in evaluating their level of function, recording findings to develop treatment programs.
• Develop and implement conditioning and rehabilitation programs consistent with the physician's Plan of Treatment and patient/rehab team goals, adjusting treatment as needed.
• Confer with physicians and clinical team members to obtain patient information and assist in developing, implementing, and revising the therapy treatment program.
• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans, supervise and evaluate their performance, and monitor documentation.
• Accurately, promptly, and thoroughly document patients' care observations, interventions, and evaluations.
• Report patient's progress to the patient's physician, Clinical Manager, staff, patient, and family, and submit evaluation, treatment plans, progress reports, and discharge summaries.

Requirements

• Current and unrestricted Physical Therapy license
• Current CPR certification
• Strong organizational and communication skills

Qualifications

• Degree from an accredited Physical Therapy Program (approved by the APTA)
• Minimum of one year physical therapy experience preferred
