Deliver high-quality physical therapy services to senior residents in Palm Beach Gardens, FL, developing individualized treatment plans to improve their function and independence.

Responsibilities

• Deliver high-quality physical therapy services to residents
• Develop individualized treatment plans to improve function and independence
• Collaborate closely with an interdisciplinary care team
• Document progress and outcomes in accordance with company and regulatory guidelines
• Evaluate patients' physical conditions, including mobility, strength, and flexibility
• Provide therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain
• Educate patients and families on exercises, posture, and body mechanics to aid recovery, prevent injuries, and offer guidance on home routines and lifestyle changes
• Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards
• Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed

Requirements

• Active Florida Physical Therapist license
• Strong communication skills and a resident-first mindset
• Degree in Physical Therapy from an accredited institution
• Valid state licensure as a Physical Therapist, or license eligible
• Strong clinical assessment and diagnostic skills
• Proficient in developing and implementing effective treatment plans
• Excellent communication and interpersonal skills with a patient-centered approach
• Ability to lift and assist patients as needed
• Ability to stand for extended periods and perform physical tasks related to therapy

Qualifications

• Degree in Physical Therapy from an accredited institution
• Experience with the senior population preferred
