About This Role

Join a stable and fun in-house therapy team at Grossmont Post Acute to provide physical therapy services to geriatric patients focused on recovery and long-term care.

Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care
• Develop appropriate care plans adhering to regulatory and clinical standards
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive devices
• Provide consultation and counseling to patients, families, caregivers, and other service providers
• Generate comprehensive discharge summaries of services provided

Requirements

• Physical Therapy license

Qualifications

• Open to Physical Therapists at all experience levels. New Grads are welcome!
