Provide home-based physical therapy services to older adults, focusing on regaining mobility and independence. This role offers flexibility and autonomy in managing patient care and scheduling.
Responsibilities
- Conduct evaluations and create personalized therapy plans to help patients regain mobility and independence
- Coordinate visits directly with patients, choosing when and where to work
- Develop and implement care plans, with regular follow-up visits to monitor progress and ensure continuity of care
- Record all patient interactions in our easy-to-use, web-based EMR system, ensuring compliance with Medicare and company guidelines
- Maintain credentials and adhere to HIPAA and company policies to ensure the highest standards of care
Requirements
- Current PT license and registration in New York
- Strong interpersonal skills to build rapport with patients and collaborate with office staff
Qualifications
- Proven experience providing adult PT, preferably in a home care setting (though not required)
Nice to Have
- Proven experience providing adult PT, preferably in a home care setting
- Familiarity with EMR systems
