About This Role

Provide home-based physical therapy services to older adults, focusing on regaining mobility and independence. This role offers flexibility and autonomy in managing patient care and scheduling.

Responsibilities

• Conduct evaluations and create personalized therapy plans to help patients regain mobility and independence
• Coordinate visits directly with patients, choosing when and where to work
• Develop and implement care plans, with regular follow-up visits to monitor progress and ensure continuity of care
• Record all patient interactions in our easy-to-use, web-based EMR system, ensuring compliance with Medicare and company guidelines
• Maintain credentials and adhere to HIPAA and company policies to ensure the highest standards of care

Requirements

• Current PT license and registration in New York
• Strong interpersonal skills to build rapport with patients and collaborate with office staff

Qualifications

• Proven experience providing adult PT, preferably in a home care setting (though not required)

Nice to Have
