A Physical Therapist (PT) will implement treatment programs to assist pediatric patients with physical, neurological, cognitive, and social/emotional disabilities or delays by planning and administering physical therapy services in the home and community.
Responsibilities
- Provide high-quality care and meet patient and family needs by performing evaluations, interpreting assessment results, developing goals, and creating/implementing physical therapy treatment plans.
- Assist pediatric patients to develop or regain functioning and improve independence by implementing skilled interventions.
- Educate, coach, and empower caregivers and families to carry over therapeutic activities into the child's daily routine.
- Coordinate with referral partners to provide services for children in accordance with physician orders.
- Assure continuation of the therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs.
- Document patient care services and care coordination in an electronic medical record system.
Requirements
- State Physical Therapist license
- Current CPR certification
Qualifications
- A minimum of 1 year of experience preferred
