Physical Therapist

About This Role

Implement and administer physical therapy treatment programs for pediatric patients with various disabilities and delays in a home and community setting.

Responsibilities

• Provide high quality care and meet patient/family needs by performing evaluations and interpreting assessment results
• Develop goals appropriate to pediatric patient and create/implement physical therapy treatment plans in conjunction with the physician
• Assist pediatric patients to develop or regain physical, neurological, cognitive, and/or social/emotional functioning
• Improve patient independence by implementing skilled interventions to maximize potential of each individual child
• Educate, coach, and empower caregivers and families to develop skills for carrying over therapeutic activities into the child's daily routine
• Coordinate with referral partners to provide services for children in accordance with physician orders
• Assure continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs
• Document patient care services and care coordination in an intuitive electronic medical record system
• Maintain patient confidence by keeping information confidential

Requirements

• State license
• Current CPR certification
• Minimum 1 year of experience preferred
