Physical Therapist

About This Role

Implement treatment programs for pediatric patients with various disabilities or delays, providing high-quality physical therapy services in home and community settings.

Responsibilities

• Provide high quality care and meet the needs of the patient and family by performing evaluations and interpreting assessment results; developing goals appropriate to child; creating and implementing physical therapy treatment plans in conjunction with the physician
• Assist pediatric patients to develop or regain physical, neurological, cognitive and/or social/emotional functioning and improves their level of independence by implementing skilled interventions to maximize the potential of each individual child
• Participate in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine
• Coordinate with referral partners to provide services for children in accordance with the physician order
• Assure continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs
• Document patient care services and care coordination in an intuitive electronic medical record system
• Maintain patient confidence by keeping information confidential

Requirements

• State license
• Current CPR certification

Qualifications

• A minimum of 1 year of experience preferred
