The Physical Therapist will plan and administer physical therapy services in home and community settings for pediatric patients with various disabilities and delays. They will develop and implement treatment programs to help patients regain functioning and improve independence.
Responsibilities
- Provide high-quality care to patients and families by performing evaluations, interpreting assessments, developing goals, and creating/implementing treatment plans in conjunction with the physician.
- Assist pediatric patients to develop or regain physical, neurological, cognitive, and/or social/emotional functioning and improve their level of independence through skilled interventions.
- Educate, coach, and empower caregivers and families to develop skills for therapeutic activities in the child's daily routine.
- Coordinate with referral partners to provide services for children in accordance with physician orders.
- Design and instruct patients, families, and caregivers in home exercise programs to assure continuation of the therapeutic plan following discharge.
- Document patient care services and care coordination in an electronic medical record system.
- Maintain patient confidence by keeping information confidential.
Requirements
- State Physical Therapist license
- Current CPR certification
Qualifications
- A minimum of 1 year of experience preferred
