About This Role

Join a compassionate therapy team in a skilled nursing facility to empower healing and restore independence for residents. This role focuses on returning patients to optimal physical function and facilitating the achievement of physician and therapy goals.

Responsibilities

• Return patients to optimal physical function
• Facilitate the achievement of patients, physicians, and therapy goals within a SNF setting
• Instruct patients in maintaining achieved level of function after discharge through education
• Instruct patients in maintaining achieved level of function after discharge through independent exercise program
• Instruct patients in maintaining achieved level of function after discharge through appropriate activity modification

Requirements

• Bachelor’s Degree or Master’s Degree in Occupational Therapy from an accredited college or university
• Current and valid license granted from state of employment
• Basic computer skills and the ability to navigate through various software programs, including service delivery software and the electronic health record software
