Occupational Therapist, PRN

About This Role

Assess, plan, and execute rehabilitative programs to restore or improve function for patients suffering from disease or injury, focusing on activities of daily living, mobility, and cognitive tasks. Collaborate with physicians to establish patient-centered treatment plans and discharge goals.

Responsibilities

• Assess, plan, organize, and participate in rehabilitative programs for patients
• Restore or improve function in activities of daily living, functional mobility, cognitive tasks, strength, coordination, and range of motion
• Collaborate with physicians to establish and evaluate treatment programs, plans of care, goals, and discharge plans
• Establish a plan of care in conjunction with the patient and/or family members
• Establish realistic discharge plan incorporating patient and/or family member goals
• Implement the occupational therapy treatment plan
• Select appropriate treatment activities to progress patient towards goals
• Revise goals/plan of care with patient/family input
• Provide ongoing patient/family education and training
• Provide discharge instructions, follow-up, and referral to community resources as appropriate
• Act as a referral source for staff, families, and physicians on services and equipment related to rehabilitation services
• Document evaluation, plan of care, and each treatment session according to departmental guidelines
• Complete and submit family education, discharge, and other required documentation within facility and program guidelines

Requirements

• Masters or Bachelors in Occupational Therapy
• Occupational Therapist license
• Basic Life Support or BLS - Instructor certification
• Effective communication skills
• Customer service focused
• Team oriented
• Basic computer skills
