Occupational Therapist (OT) - Home Health

About This Role

Provide occupational therapy to home health patients, helping them regain daily function and independence. Work as part of a multidisciplinary team to implement personalized care plans and ensure patient safety within their home environment.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury
• Work as part of a home health team (RN, LPN, CNA, HHA, PT, SLP) focused on the patient’s plan of care and personal goals
• Assess patient, observe deficits, establish therapy goals and document progress
• Assist patient with exercises to improve fine motor skills and coordination
• Suggest adaptive equipment such as grab bars and shower chairs
• Assess fall risks and introduce strategies to improve home safety
• Educate patient and family on plan of care, exercises, goals and self-care

Requirements

• Graduate of an accredited Occupational Therapy Program
• Active OT license in New Jersey
• Minimum of 1 year of occupational therapy experience, ideally in home healthcare
• CPR certification (must be achieved within 60 days)
• Knowledge of state and federal home health regulations
